Advanced Laser Proctology

LIFT

The LIFT Procedure involves Ligation of the Intersphincteric Fistula Tract. It is a relatively new technique for treating anal fistula that avoids cutting the anal sphincter .The procedure was developed by Thai colorectal surgeon, Arun Rojanasakul, Colorectal Division Department of Surgery, Chulalongkorn University in Bangkok,Thailand. Dr Gokul has trained under Dr. Rojanasakul and has successfully performed LIFT procedures for patients with complex intersphicteric fistulas.

Piles are anal venous channels that get swollen because of prolonged straining during defecation. Piles or haemorrhoids can present with variable symptoms like bleeding during defecation, itching, and swelling in the anal verge which may prolapse during defecation.
There are many causes for piles such as prolonged constipation, avoiding fibre diet, sitting for a long time on the toilet, and rarely piles can be due to rectal cancer.
Sclerotherapy, banding, infrared coagulation, laser therapy, stapled haemorrhoidectomy, and conventional open surgery.
Fissure-in-ano is also known as anal fissure. Fissure is the tear in the mucosal lining of the anus. It presents with painful defecation with a streak of blood along with the fecal matter. The patient gives a history of passing hard motion before the onset of pain.
Passing hard motion is the main cause of anal fissure. An anal fissure can also present in some inflammatory bowel diseases like Crohn’s, post-delivery, and during recovery of any illness.
Fistula-in-ano is an abnormal communicating tract between the anal canal and exterior skin. The patient typically gives a history of painful swelling or a boil or a cavity containing pus that burst to the exterior near the anus by discharging pus or blood mixed fluid. It is one of the disease conditions which occurs frequently in spite of treatment.
The most common cause for an anal fistula is the infection of the anal gland forming an abscess (cavity filled with pus) and bursting to the exterior. Other causes are radiation, sexually transmitted diseases, tuberculosis, inflammatory bowel disease like Crohn’s disease, and diverticulitis of the rectum.
It is a daycare procedure and no need for admission. It can be done under mild sedation, painless, scarless, and sutureless. There won’t be any surgical wound, and the patient can resume back to work as soon as possible.
Most of the anal conditions like piles and fissures are due to lack of fibre rich diet, prolonged constipation, sitting for a long time at the toilet, lack of adequate fluid intake, and sedentary lifestyle. If the patients have these risk factors even after treatment, then the patient can again develop these conditions. Fistula is a separate entity due to infection of anal glands, as infection can recur again and so does fistula.
One can keep anal diseases away by following the following methods
Adequate fluid intake.
Eat fibre rich diet.
Exercising daily.
Avoid sitting for a long time in the toilet.
Maintaining proper hygiene.
