Minimal access surgery or laparoscopic surgery is a newer innovative technique of performing abdominal surgery with the help of specialized equipment called laparoscope and hand instruments which will be passed inside the abdomen through minute cuts. The laparoscope is a slender tube with inbuilt camera which transmits video image of the abdominal cavity to the monitor kept besides patient. Surgeon operates with the help of instruments by seeing the monitor.

Laparoscopic surgery has transformed  the field of GI surgery. Today surgeons operate basic appendectomy to much complicated oncological surgeries and bariatric surgeries by laparoscopic approach. Laparoscopic surgery has revolutionized the field of surgery interms of patients’ recovery as well as cosmetic out look. Laparoscopic surgery has many advantages like early mobilization, lesser pain, lesser hospital stay, early resumption of work, lesser wound related issues, and better cosmetic results.
Hernia is the protrusion of the intestine or the omentum through the defect in the muscle. There are many types of hernias like inguinal hernia, umbilical hernia, incisional hernia. The main cause of hernia is increased intra-abdominal pressure due to prolonged constipation, chronic cough, and straining while urination. Females are also prone for umbilical hernia after delivery as pregnancy causes stretching of abdominal muscles leading to muscular weakness. Hernia should be addressed as soon as possible as the protruded intestine is at greater risk for obstruction and vascular compromise leading to gangrene.
Patients will be advised not to lift weights for 2 to 3 months to avoid undue stretching of abdominal muscles and not to do abdominal exercises. But activities like walking, climbing stairs and minimal work can be continued.
Mesh is a synthetic material made up of polypropylene material. It provides strength and reinforcement to the weakened muscles and prevents recurrence.
Recurrence of hernia is due to extreme muscular weakness, diabetes mellitus, poor general condition, malnutrition, presence of precipitating factors like chronic cough, constipation, urinary disturbances. The main reason for recurrence of umbilical hernia in females is not doing abdominal muscle strengthening exercises after delivery. We always advise abdominal muscle strengthening exercises two months after hernia surgery to avoid recurrence.
Gallbladder stones are caused by multiple factors like infection, increased cholesterol in bile, poor functioning of gallbladder and hereditary factors. Gall bladder stones are more common in females, at the age of 40, after pregnancy and obesity.
Gallbladder acts as a storehouse. About 1 to 1.5 litre bile is secreted from the liver daily and it gets stored in the gallbladder. During the digestion process, a hormone called cholecystokinin pancreozymin is released which induces contraction of gallbladder releasing bile in the duodenum. Although it seems that gall bladder plays a small role in the digestion process, it is very insignificant and removal of it will not cause any trouble in digestion.
Poorly controlled diabetes mellitus, coronary artery disease, hypertension, chronic cholecystitis, choledocholithiasis, presence of cholangitis and pancreatitis make gallstone disease complicated.
Jaundice is yellowish discolouration of sclera, nail beds and urine. Stones in the gallbladder can migrate to the common bile duct causing the blockage, which leads to collection of bile in the biliary system and get absorbed in the blood stream leading to jaundice. Presence of jaundice in a gallstone patient warrants emergency hospitalization to rule out cholangitis and needs prompt treatment to avoid further complications like septic shock. After initial resuscitation, the patient will need MRCP to confirm the presence of stone in the common bile duct. ERCP will be carried out to retrieve stone. Once the stone is retrieved and the patient is stabilized, laparoscopic cholecystectomy is carried out to remove the gallbladder.
Appendix is a vestigial organ situated at the junction of small intestine and large intestine in the right lower part of abdomen. There is no proven function of appendix in the human body. Appendix can get inflamed suddenly due to blockage of its lumen either due to fecolith or secretions. Patient will develop pain around the umbilicus initially and later radiates to the right side of the abdomen. Nausea, vomiting and mild fever will be the associated symptoms.
